In 2023, Achievable Dream was founded with a clear purpose: Achievable Dream, LLC’s mission is to provide excellent service to individuals with numerous disabilities that will allow them to live a fulfilling life and develop their independence.We believe in providing compassionate care that goes beyond the basics, focusing on positive community integration through local events, activities, and celebrations.

Enhancing the lives of those we serve. We are committed to using a Person Centered Approach to care. We believe in taking the time to understand an individual’s behavior and interest so that we can shape the care provided to address all needs.

At Achievable Dream, we believe that everyone is a respected and valued member of our Dream Team. We carefully select and train our caregivers, ensuring they are passionate, educated, and ready to make a positive impact of the lives we serve. We take time to understand individuals and who they are so that we can better provide for their needs.

Attendance

Every individual that we serve is entitled to a certain amount of quality care. It is our duty that each individual received high quality care each day without exception.

Person Centered Approach

Person-Centered Planning promotes inclusivity, independence, and rapport by using a collaborative process to individualize goals and plans, so they connect with the person’s unique strengths and values. In addition, these processes improve outcomes and engagement as people feel more self-determination in their care. For staff, this manifests as conversations about motivations, growth, and goals that become part of the plan. This class provides practical tools for using Person-Centered approaches to create goals and write plans and various documents across the helping professions.

Positive Behavior Support

Positive behavior support (PBS) strategies are considered effective when interventions result in increases in an individual’s success and personal satisfaction, and the enhancement of positive social interactions across work, academic, recreational, and community settings. Valued outcomes include increases in quality of life as defined by an individual’s unique preferences and needs and positive lifestyle changes that increase social belonging.
